About our client: 
An established and expanding Case Management Company working in the Medico-legal sector is currently recruiting an Associate Case Manager to join their dynamic and very supportive team of professionals, who are dedicated to improving their clients’ lives and enabling them to reach their goals. Our client works with children and adults with acquired / traumatic brain injury, spinal injury, limb loss, dementia, and polytrauma across London and the Southeast Counties. 
Case Management is a challenging but extremely rewarding role and is both varied and collaborative.  An Associate Case Manager supports a Lead Case Manager who is instructed by the legal sector to support a client during litigation and in some instances by a financial Deputy, post settlement. They are responsible assessment, coordination and management of rehabilitation services and care packages. The aim is to develop individualised rehabilitation goal-based programmes that support clients in regaining as much independence as possible and achieving their full potential. There is a comprehensive induction, training plus a mentorship program to ensure you are well supported. You would be joining a company who foster a positive team dynamic and who take care of each other.

Overview of role:
The role of Associate Case Manager requires you to complete a detailed initial assessment of the care, rehabilitation, and case management needs of a client, under the supervision of the Lead Case Manager and in collaboration with other professionals. This includes meeting with families to ascertain need and progress, preparing rehabilitation programmes, risk assessments, and care plans and outline a clear case management plan and objectives for that client. The Associate Case Manager will provide ongoing case management support to the client, their family, and care team, and liaise with therapists, medical practitioners, solicitors, social services, clients’ employers and schools. Most importantly you will act as an advocate for your client and ensure they are receiving the right support in all aspects of their care plan. This is a self-employed role offering £25.00 - £30.00 gross per hour, depending on experience. Your caseload will build as your knowledge of the role increases.

Who this job would suit:
Our client is looking for a candidate with experience within the disability/care sector, in a senior role. This is a client-facing role so you must be a people-person, have an engaging personality, and be extremely resilient, and tenacious. As you will be managing your own caseload, you must be organised and confident in working independently.  Excellent communication, IT, and written skills are essential. The position requires you to travel to clients’ homes and to various appointments, a driver with their own car is essential.
